為使病人出院後能接受更妥善的照護，避免再急診及再住院，醫院會提早為出院後仍有照護需求的病人，規劃出院準備及出院後追蹤諮詢服務。

對於有銜接後續醫療或長照服務者，出院前會由專業人員評估照護需求，協助出院後轉介長照服務、居家醫療整合照護、各類居家療護、養護機構或護理之家照護等。健保的用心，讓您出院後更放心！
